Transitions of Care: Connecting Patients, Families and Physicians

When you’re not feeling well, where do you want to be? Home, of course... Home is always associated with comfort, security and posi­tive feelings. St. Mary’s of Michigan and the Michigan Ministries of Ascension Health now have an option to care for you at home, Reverence Home Health & Hospice. It provides home health, hospice, infusion services, tele-health monitoring and palliative care services across the state. In preparation for health care reform and population health management, Linda Young, President of Reverence Home Health & Hospice will share how this new company will ensure seamless transitions of care and will connect patients, families and physicians to provide the highest quality of care and experiences.

Beyond Bandages: When Wounds Refuse to Heal

Presenters: St. Mary’s of Michigan Wound Care Team

Broken skin can be acute or chronic and treated with bandages or prescription medications, but a majority of wounds - often chronic – may not get the proper care needed. Chronic wounds most frequently occur in individuals with diabetes, vascular disease or those who have undergone radiation therapy for cancer. The new Wound Care & Hyperbaric Center offers the latest treatment and technology for those hard to heal wounds in a convenient, outpatient setting. Located at our Towne Centre location, near Fashion Square Mall, the Center houses two state-of-the-art hyperbaric oxygen chambers which promote healing for chronic or hard-to-heal wounds. Our Wound Care Experts will share how we can bring comfort, healing relief and specialized care to improve the healing process.

Standing Tall Against Falls

Presenter: Kelli Jankens, Injury Prevention Coordinator

Do you know that one in three older adults fall each year? Falls are the most common cause of hospital admissions for trauma and the leading cause of injury deaths among older adults. The good news is that most falls can be prevented. By identifying your own personal risk factors and eliminating as many of them as possible, you can greatly improve your chances for staying independent and on your feet. Don’t succumb to the common myth that “falling is just a normal part of growing old.” In fact, falling is not a normal or inevitable part of get­ting older. People fall for many reasons and usually it is a combination of risk factors that led to the fall. Kelli will help you identify and check all of your “fall-ty” behaviors.

Our displays from 8:30 - 10 a.m. will feature information about organizations which offer educational and fun things to do during the LONG Michigan winter.  Past exhibitors have included local muse­ums, nature centers, theaters, clubs, etc. Our corporate sponsors will mingle with our members and guests and share infor­mation about their products and services.  OLLI members who sell arts & crafts will have an opportunity to share their wares.

The SVSU Osher Lifelong Learning Institute is a member-based organization that offers non-credit educational programs & travel opportunities for the over-50 population.  It is currently in its twelth year of operation and has more than 1400 members.  OLLI Monthly Membership meetings are held at the Saginaw Valley State University.
